Health Insurance – Medical insurance and prescription drug benefits are defined in Page 70 of the current contract agreement. UUP Contract
Empire Plan (NYSHIP) – New York State Health Insurance Program administers medical insurance and prescription benefits. Website includes participating provider directory search.
Benefits Trust Fund Booklet — Benefits Booklet provides information about dependent scholarship program, dental and vision care, and life insurance.
Davis Vision – Vision care provider information and claim form are available online.
Delta Dental – Dental care is provided by Delta Dental. Website includes online provider directory search. Highlight sheet gives an overview of services for UUP members. Claim forms are also available online.
Emergency Travel Assistance – Access to emergency travel assistance for UUP bargaining unit members and their families, offered through the UUP Unum Provident life insurance benefit.
