Here are some quick pointers to help you with questions you might have regarding the UUP Benefit Trust Fund (BTF) and your benefits:

2. Retired UUP members may call Anne Marine, retiree member services coordinator, at (800) 342-4206.

3. BTF information can be accessed on the UUP Web site, www.uupinfo.org. Click on Benefits for various options.

4. Our dental provider is Delta Dental (effective April 1, 2008). Delta’s toll free number for UUP members is (800) 471-7093 and the Web site is www.deltadentalins.com.

5. Our vision provider is Davis Vision. Davis Vision’s toll free number is (800) 999-5431 and its Web site is www.davisvision.com.

6. When you call the Benefit Trust Fund, please have your identification number ready. This number can be found on your NYSUT membership, Delta Dental or Davis Vision cards. The number is the same on all three.

7. Eligible UUP members must be enrolled in the Benefit Trust Fund in order to receive benefits. This means that the BTF must have an enrollment card on file for you that also lists your eligible dependents. The enrollment form can be found at www.uupinfo.org. 8. We need to have up-to-date Student Verification forms. Your unmarried dependent children, ages 19-25, are eligible for dental and vision benefits if they are full-time students. Without the proper documentation on file, your dependents will be denied dental and vision coverage. Additional information can be found at www.uupinfo.org. 9. Scholarship applications must be postmarked within 60 days from the last day of the semester for which the dependent is applying. Transcripts need to be requested directly from the school and must contain the correct semester information. Additional information can be found at www.uupinfo.org. 10. Finally, please tell us if you have moved, been married or divorced, or have added or deleted any dependents. These forms can be found at www.uupinfo.org. Click on Benefits, Benefits Forms and either use the BTF/UUP Member Change of Address Card or the BTF Change of Marital or Dependent Status forms.
